Discharge Planner
Our discharge planner, in consultation with the attending physician and nursing staff, will assist in planning the patient's post-hospital care and coordinating patient and/or family teaching when required. The Discharge Planner will be available to help with any needs or questions you may have regarding community resources such as home care, special equipment and long term care.

Chaplains
Volunteer chaplains are available to visit with patients and families, and to provide support in times of crisis.
